Overview

Adelaide PHN is investing in innovative and integrated care models as a means to meet the challenges currently facing our health system.

Whilst elements of Integrated Care underpin all Adelaide PHN programs and commissioned services, our Integrated Care approach is led by three flagship programs – HealthPathways South Australia, Care Connections and Health Care Homes.

Background

Our health system is facing a challenge - half of all Australians are estimated to be living with at least one chronic condition, and one quarter are living with two or more 1.

Charged with the task of driving improvements to the local health system, Adelaide PHN is investing in innovative and integrated care models in order to better meet the needs of people living with chronic and complex conditions locally and to support the building of a more effective local health system.

Our Integrated Care Approach

HealthPathways South Australia, Care Connections and Health Care Homes are all innovative and tailored programs which combine local knowledge with best practice frameworks. Together they form the Adelaide PHN Integrated Care Approach.

  • HealthPathways South Australia is an online portal – designed for use at the point of care – which provides GPs and other health professionals with condition-specific pathways of care and localised referral options
  • Health Care Homes is a Commonwealth Government Trial being delivered across Australia by various PHNs where participating general practices are supported to provide better coordinated and more flexible care for people living with chronic and complex conditions
  • Care Connections is an Adelaide PHN-designed program, being delivered in areas of high need across Adelaide, whereby participating general practices receive dedicated support via a suite of activities to improve coordination and collaboration between care providers and to assist these practices to work with the Patient Centred Medical Home (PCMH) model
