HealthPathways South Australia is being delivered via a partnership between SA Health, Adelaide Primary Health Network and Country SA Primary Health Network.

The HealthPathways South Australia team includes: metropolitan and country based General Practitioner (GP) Clinical Editors, Clinical Coordinators and Clinical Leaders from across the primary and acute health sectors. GPs, medical specialists, nurses, allied and other health associated professionals are actively engaged to collaborate in the localisation of each of the portal’s condition-specific pathways of care.

Tailored to the local context, HealthPathways South Australia enables health professionals across the primary and acute sector to work together to improve access for people; ensuring they receive the right care, at the right time, in the right place.

Whilst condition-specific pathways will continue to be developed across the 2018 year, the portal is expected to go live – and be available to local health professionals from early in 2018.
