HealthPathways South Australia one of three flagship programs making up the Adelaide PHN approach to integrated care.
HealthPathways is free and provides GPs and other health professionals with access to condition-specific pathways, relevant patient resources, and local service information. The portal is designed to be used at the point of care by GPs but is also available to specialists, nurses, and allied health professionals across South Australia. Tailored to the local context, HealthPathways supports health professionals to provide consistent care and management, enhancing the patient journey through our local health system and ultimately improving health outcomes.
